Q) What is Prepaid Health
Clinic (PHC)?
A) Prepaid
Health Clinic is an organization
authorized to provide basic
medical services to persons
enrolled with the organization
on a prepaid per capita
basis including those basic
services such as Primary
Care Physician, ambulatory
diagnostic treatment at
the Primary Care Physician
site, and preventive health
care.
Q) What does PCP
mean?
A) PCP
means Primary Care Physician,
your main doctor, elected
in coordination with the
Agent that filled out your
application form with you.
Your PCP is who you go
for any medical care need
and who appears on your
I.D. Card.
Q) What do I need
to do to make an appointment
with my Primary Care Physician?
A) You
may choose to do the following:
Personally call your PCP
at the Phone number which
appears on your ID card
or
If you have any difficulty
in contacting your PCP,
please call our Customer
Care Department at 1-866-PROMEDI.
Q) How much should I pay
when going to my PCP?
A) You
only have to pay $ 10.00
each time you go to your
PCP.
Q) How much should
I pay when taking my child
who is less than one (01)
year old?
A) You
must pay $ 15.00 at time
service is rendered.
Q)
How long does it take to
have my first appointment
after becoming a Member?
A) It
could take as short as
24 hours depending of the
schedule of each PCP. Normally
in 48 hours you are helped
by your Primary Care Physician.
Q) Who is eligible for
ProMedical Plan?
A) Eligible
individuals are the spouse,
domestic partner and children
of Primary Subscriber.
Q)
How do you select your
Providers in your Plan?
A) All
Providers have been thoroughly
interviewed and selected
according to their specialties
in the medical field. ProMedical
Plan. performs on-site
evaluations of each prospective
Medical Center or Physicians
Office and medical doctors.
We have assured that they
meet or exceed the standard
requirements of our Quality
Assurance Program. Each
Physician is evaluated
on the following criteria:
Experience and Qualifications;
Credentials: Current Florida
Board of Physicians, DEA
license and malpractice
insurance; Good standing
with Department of Professional
Regulations.
Q) If I refer a
person to your company,
do you pay me something
for that referral?
A) Yes,
ProMedical Plan. appreciates
all referrals from any
member and rewards that
member with a $ 40.00 check
for each referral received.
Q Up to which age
can I have my child in
ProMedical Plan?
A) Up
to age 23 if he or she
is living with Primary
Subscriber and is a Full-time-student.
Children beyond the age
of 23 with mental or physical
handicapped may be in ProMedical
Plan.
Q) When are my monthly
payments due?
A) Payments
are due in advance every
month by the first of the
month.
Q) What are Advance
Directives?
A) Advance
Directives – The
Patient’s Right to
Decide, are documents that
advise you of your right
to decide and make decisions
concerning your own health,
including the right to
refuse treatment. An Advance
Directive is a written
or oral statement about
how you would want medical
decisions made if you were
not able to make them yourself.
These include three types
of decisions you can make
in advance: 1.) Living
Will, 2.) Health Care Surrogate
Designation, and 3.) An
Anatomical Donation. The
forms for your review and
discussion with your family
and physician are available
to you by calling Customer
Service.
